MEANINGFUL USE
What follows are the 15 “core” measures which must be met by eligible providers to qualify for HITECH stimulus funding.
In addition, eligible providers must choose and report on five (5) “menu set” objectives of which one (1) public health objective must be included.
Eligible providers must also report on a total of six (6) clinical quality measures; three (3) of which are required by specialty and three (3) which are selected as alternative core measurements.
CORE REPORTING OBJECTIVES AND MEASURES
Must meet all 15 “core set” reporting measures.
- Computerized physician order entry (CPOE) for medication orders for more than 30% of patients.
- E-prescribing for more than 40% of patients.
- Report on six (6) clinical quality measures (CQM) to CMS/States (provide aggregate numerator and denominator through attestation).
- Implement one (1) clinical support rule.
- Provide patients with an electronic copy of their health information, upon request for more than 50% of patients.v
- Implement functionality for drug-drug and drug-allergy interaction checks.
- Record patient demographics on more than 50% of patients.
- Provide clinical summaries for more than 50% of patients for each office visit.
- Maintain an up-to-date problem list of current and active diagnoses for 80% of patients.
- Maintain active medication list for more than 80% of patients.
- Maintain active medication allergy list for more than 80% of patients.
- Record and chart changes in vital signs for more than 50% of patients aged 2 years and older.
- Record smoking status for patients 13 years and older for more than 50% of patients.
- Perform at least one (1) test to ensure capability to exchange key clinical information among providers of care and patient-authorized entities electronically.
- Conduct or review security risk analysis and implement security updates as necessary to protect electronic health information.
MENU SET OBJECTIVES
Must Report on Five (5)
- Implement drug formulary checks to at least one for the entire reporting period.
- Incorporate clinical lab test results as structured data for more than 40% of tests ordered.
- Generate list of patients by specific conditions.
- Send reminders to patients (per patient’s preference) for preventative and follow up care for more than 20% of patients 65 and older or 5 and younger.
- Provide patients with timely electronic access to their health information for more than 10% of patients.
- Use certified EHR technology to identify patient-specific education resources and provide to patient, if appropriate for more than 10% of patients.
- Perform medication reconciliations for more than 50% of transitions of care..
- Provide a summary of care record for each transition of care/referrals for more than 50% of patients.
- Record advance directives for patients 65 years and older for 50% of patients.
- Perform at least one (1) test to submit electronic data to immunization registries/systems*.
- Perform at least one (1) test to submit electronic syndromic surveillance data and reportable lab results to public health agencies*.
*Must report on one (1) public health objectives.
